I am aware of
your policy to conduct reference surveys and certifications on all credit
applicants which may include a credit report. I understand that I am
responsible to pay for products and services rendered. I understand
that if a payment becomes past due, a compensating charge of 2% annually or
minimum of $.50 will be due on delinquent amounts from the date payment was
due. In the event of default, I may also be responsible for reasonable
